Children (Ages 3 and above)
Due to limited availability I can see children (ages3+) if physicians, psychologists, or insurance care managers submit a referral to me so I can review it if if is appropriate.
Appointments for children/adolescents
Once a referral is submitted and accepted electronic intake forms are sent via email then behavior ratings are completed by the parents and an intake interview is conducted to obtain background information and the presenting concerns for children and adolescents. Play therapy then can commence after a treatment plan is discussed with parents or guardians.
Clients who want to schedule an in-person or telehealth remote appointment can click the red “Request Appointment” button at the top of the page using Simple Practice.
Please complete and email or fax to our office if you would like to refer a patient.
If no availability is open, referral will be placed on a wait list.
