Sorry, Aloha Care, Tricare, and Kaiser insurance are not accepted.
Patients are responsible for co-payment fees and any service fees if insurance claims are denied or services not covered by their insurance plans. If you are not certain about your co-payment fee for services, please contact your insurance company to a quote. We are not able to give you a co-pay cost estimate as each insurance company has different rates.
Missed appointments and cancellations (without 24 hour notice) are charged a “no show fee” that is not covered by their insurance plans
Insurance will be billed according to customary and usual rates according to your health plan.
Cash payments are accepted. Sorry, No sliding fee.
All major credit cards via Simple Practice Client portal are used as form of payment and required for billing co-pays.
Questions about fees can be discussed prior to any appointments.
Any legal fees are the responsibility of the patient and family.
